1. When completing the form leave the APPLICATION NUMBER blank. If you are filling in the form for someone else you must put your details against title, surname, forename and initials. Fill in your contact details.

2. Exam information. Tick ABRSM Centre.

2a. Centre information. Insert the Centre Name and Centre Code. ( to find these details click on Exam centre codes below ).

As this is a theory application you do not need to complete 2b, 2c or 2d.

3. Complete candidate information. SUBJECT CODE FOR THEORY IS 99

4. Complete the declaration.

5. Complete payment section.
